What to Bring to an Interview
All applicants should be able to complete an application, which includes references, dates of employment and history of education. With reference to the individuals with disabilities, his or her disability is taken into consideration when reviewing the application process.
An individual with a disability must provide the appropriate documentation in reference to their disability prior to being interviewed. They can refer to the form “Required Documentation.” This describes the type of documentation required for qualification.
The individual can bring the documentation in to the office, have the provider fax it, or mail it to the corporate office. The documentation will be reviewed, and if appropriate, the individual will be called for an appointment with the Human Resources Director.
The following is the required documentation for the specific disabilities that we encounter most often. However, we will attempt to accommodate a variety of disabilities. Psychiatric evaluation, Axis I included.
Psychological evaluation. This would include test results such as WISC-R, WRAT-2, and Bender-Gestalt.
Medical report. Please include restrictions and accommodations.
Note: All documents must be signed by a doctor.
An individual without a disability can fill out an application any time between 9:00 a.m. and 4:00 p.m., Monday through Thursday, and 9:00 am to 3:00 p.m. on Fridays. We do look at a person’s prior experience for the position to which they are applying. In the event there are no positions open, the applications will be kept on file for the managers to review when there is a need.
